How does Western medicine treat this?
1. Local infiltration: Infiltrate the area around the swelling with 5-10 ml of 1% procaine and 12.5 mg of hydrocortisone acetate, once a week for 5 weeks as one course of treatment. If it is inflammatory, add 40,000 units of gentamicin or 0.5 g of kanamycin to the procaine solution for local infiltration, once a day for 5 weeks as one course of treatment.
2. Spermatogenesis Inhibition: Gossypol acetate 20 mg once daily for 2 months can be taken orally, then 40 mg once weekly. This can temporarily inhibit spermatogenesis and reduce epididymal congestion. Blood potassium levels should be monitored during medication; if potassium levels are too low, medication should be discontinued.
3. Local infrared irradiation: once a day, 10-20 minutes each time, for 7 days as one course of treatment.
4. Surgery: For severe epididymal stasis or chronic stasis, especially with obvious stasis, epididymal seminal granuloma, or stasis accompanied by inflammation, epididymectomy or partial vas deferens resection should be performed.
How does Traditional Chinese Medicine differentiate syndromes and treat diseases?
1. Damp-heat retention type: Symptoms include swelling and pain in the epididymis, pain radiating to the waist and thighs, soreness and discomfort in the perineum and lumbosacral region, or accompanied by chest and rib distension, bitter taste in the mouth and poor appetite, or chills, fever, scanty and dark yellow urine, red tongue with yellow and greasy coating, and wiry and rapid or slippery and rapid pulse.
Treatment principle: Clear heat and promote diuresis, resolve blood stasis and promote sperm production.
Prescription: Gentian root 10g, Gardenia fruit 10g, Scutellaria root 10g, Bupleurum root 10g, Oldenlandia diffusa 20g, Verbena officinalis 15g, Dandelion 15g, Red peony root 10g, Angelica sinensis 10g, Akebia stem 10g, Plantago seed (wrapped) 15g, Achyranthes bidentata 15g, Jade powder (wrapped) 20g, Litchi seed 10g, Sichuan chinaberry 10g, Sparganium rhizome 10g, Salvia miltiorrhiza 15g.
2. Obstruction of essence and blood stasis type: Symptoms include scrotal swelling, epididymal swelling or hard nodules, severe pain upon touch, the pain is continuous or unbearable, the pain is worse at night, the pain radiates to the waist and abdomen, or the scrotal skin is dark purple, which is aggravated by fatigue or sexual intercourse, the tongue is dark or has petechiae on the sides, and the pulse is choppy.
Treatment principle: Promote blood circulation and dissipate nodules, remove blood stasis and promote spermatorrhea.
Prescription: Bupleurum 10g, Angelica sinensis 10g, Paeonia lactiflora 10g, Astragalus membranaceus 15g, Ligusticum chuanxiong 10g, Sparganium stoloniferum 10g, Curcuma zedoaria 10g, Prunus persica 10g, Carthamus tinctorius 10g, Ostrea gigas (decocted first) 30g, Achyranthes bidentata 10g, Eupolyphaga sinensis 10g, Fritillaria thunbergii 10g.
3. Kidney deficiency and cold coagulation type: Symptoms include epididymal swelling and dull pain, lower abdominal distension, aggravated by cold and relieved by heat, aversion to cold, scrotal contraction, decreased libido, pale tongue, thin white coating, and deep, thready and slow pulse.
Treatment principle: Warm the meridians and soften hardened masses.
Prescription: Angelica sinensis 10g, Lycium barbarum 10g, Cinnamomum cassia (added later) 10g, Lindera strychnifolia 10g, Poria cocos 10g, Foeniculum vulgare 10g, Zingiber officinale 10g, Aconitum carmichaelii (decocted first) 10g, Aquilaria sinensis powder (taken in divided doses) 4g, Glycyrrhiza uralensis 5g, Jujube 20g.
Traditional Chinese medicine prescriptions for treating epididymal congestion
1. Peach kernel 12g, rhubarb 12g, cinnamon twig 6g, Glauber's salt 10g, licorice root 6g. Decocted in water and taken orally, one dose per day.
2. Take 30 grams of Sophora flavescens, 30 grams of Gentiana scabra, 15 grams of Scutellaria baicalensis, 15 grams of Phellodendron chinense, 20 grams of Alum, and 20 grams of Poria cocos. Decocted in water, use the hot decoction to fumigate and wash the scrotum. One dose per day, for four fumigation and washing sessions.
3. 10g of wolfberry, 12g of Sichuan pepper, 10g of green tangerine peel, 10g of dried tangerine peel, 10g of red peony root, 10g of alisma rhizome, 6g of dipsacus root, 10g of trogopterus dung, 10g of mantis egg case, 10g of dioscorea hypoglauca, and 10g of madder root. Decoct in water and take twice daily, morning and evening, one dose per day.
4. Grind 30 grams of white mustard seeds, 30 grams of Sichuan lovage rhizome, and 30 grams of radish seeds into a fine powder. Mix with aged vinegar and apply to the affected area. Change the dressing once a day.
5. Take 30g of Sophora flavescens, 30g of Gentiana scabra, 15g of Scutellaria baicalensis, 15g of Phellodendron chinense, 20g of Smilax glabra, and 20g of alum. Decoct in water and use the liquid to fumigate and wash the scrotum. Use one dose daily for four days.
6. 15g each of Ligusticum chuanxiong, Zhelan, Leonurus japonicus, Artemisia capillaris, and Verbena officinalis. One dose daily, decocted in water, used to fumigate and wash the scrotum while still hot.
7. 10g of Sichuan pepper, 10g of lychee seed, 10g of red peony root, 10g of wolfberry fruit, 10g of dioscorea hypoglauca, 10g of euryale seed, 6g of gentian root, 6g of green tangerine peel, 6g of dried tangerine peel, 6g of fennel, and 3g of licorice root. Decocted in water and taken once daily.
